O Que Tem De Errado Nesse Codigo ? Nao recebo email do formulario.

Alguem poderia me socorrer, sempre que eu crio um formulario dentro de duas layout table eu tenho problemas no recebimento dos dados, não estou recebendo o email com os dados. Vlw !
